APPLICATION FOR RE-ISSUE OF KEYS AND CERTIFICATES
An application for re-issue is made following the revocation, expiry (non-renewal of keys and certificates) or cancellation of the application (non-activation of the keys and certificates).
APPLICATION MADE WITHIN THREE (3) MONTHS OF THE DATE OF REVOCATION, EXPIRY OR CANCELLATION OF THE APPLICATION
The applicant must complete the subscription form by checkmarking Section 5 - Application for re-issue of keys and certificates ($50 plus tax) and fax the form to Notarius. In this case, the applicant only has to fill out Section 9 – Certificate because he or she is not required to appear before an IVA. Notarius will confirm the applicant's professional status with the Order before issuing a digital signature.
